My soft pad, much used, won't grip the paper like it used to.  P120 and up it works OK, but the more coarse discs slip.

Did I use it too hard and cause the problem?  Does friction heat ruin the hooks?  Can just the pad part be swapped?  Is there a fix, or am I out the cost of a new pad?

Gene,
Heat can decrease the abrasives ability to stick to the pad.  Over time the pad can heat up and melt the plastic hooks that make up our Stick Fix.
There are  a couple questions you could answer that may help to prolong the life of the pad.
What kind of sanding were you doing?
Are you using dust extraction?
Which type of abrasive were you using?

Just normal wear and tear. Velcro doesn't have eternal live.

Ofcourse there are things that can greatly speed up a pad's demise. I once was sanding and didn't understand why the paint didn't come off. On closer inspection I found I had lost the sanding disc and was sanding with bare velcro. So that meant a trip to the store. Not a Festool sander luckily, so it didn't break the bank.

I don't know if there is a way to remove the worn out velcro and glue some new on. You'd have to do a darn good job to get that smooth. I would advise to get a new pad.
