Table Saw Extension Table

I have a 10" 3hp Jet special edition cabinet saw w/ a incra TS-lll fence that I purchased a couple months ago and I now am just getting around to setting it up.  I want to build a right side extension table for it.  Does anyone have any plans or links for something like this?  Can you offer any advice?  I'm just looking for a very basic extension table.  The only reason why I am asking for plans is because it's seems odd to build the table with a 3/4" thick top and still have room to mount it to the table saw.  The existing bolt holes in the table top where the right extension wing would go are only 1 1/4" down from the top, If I use a 3/4" top that leaves me pretty much no room to drill a hole for the bolt plus the diameter of the bolt head.  So it seams that there may be a special way to connect the extension table to the table itself.

I'm just guessing but it sounds like they planned for the extension to hook on using slots with the opening facing toward the floor.

Edit: If I understand you correctly, you have 1/2" radius to work with. What size bolts are to be used?

couldn't you make the cleat oversized, like 1 1/2 X 1 1/2. drill the holes to 1" depth. then take a 3/4 x 3/4 rabbet out to accept the plywood? your mounting hardware will then be able to be beyond the plywood with still enough heft to hold it.
