I found an online source for cable sleeving in the US with a wide selection and great service.
I've used them for many items and they always ship fast and have an easy to navigate website.
I found their prices reasonable. For instance I looked at 9284K7 - Expandable Polyester Sleeving,

I was going to try the heat shrink to secure the ends but couldn't find a large enough diameter locally. I tried some of the Rockler extreme tape that I had left over. http://www.rockler.com/x-treme-tape-for-dust-joints.
It's only been a couple of weeks but is holding up well and isn't sticky.
