The drawer slides are from Amazon, search for "KV Economical Full Extension Drawer Slide 16" 100lb Load Rating" I paid the $5.94 and with a 10% coupon. They are fine for this purpose, I would not use them for anything other than shop drawers however.

 
Sweet thanks for the pics. Is there a reason for the cut out in the bottom of the shelve?

The shelves are assembled with left over scrap from the sides. Would be easier to just cut full size but this being a budget project I just cut the scrap and assembled with pocket screws.
