It seems to me that the FOG could be well served by such a thread, as it is something that all power tool and hand tool users deal with on a constant basis, allowing for a place for FOG members to discuss measures taken to avoid career-ending injuries, environmental-ocupational hazards, and general workplace safety. One problem I could see arising from such a thread would be the discussion of modification/ disabling of safety devices. A short "Please read"caveat/ disclaimer might help keep things tidy. 
Just a thought.

OK, here are three rules that I follow religiously that work well for me.

1) Unplug at power tools (including both machines and hand held tools) immediately after use.

2) Take a brief moment to think about and plan each task that utilizes a machine before performing that task.

3) When working with noisy machines, lock the workshed door in order to avoid interuptions that might startle me.

- Always let a tool spin down before putting it down.
- Always unplug the cord when changing bits. Especially with the router and the circular saw.
- Don't wear jewelry like rings, necklaces or watches. Most of the time, don't wear gloves either.
- Wear eye, ear and breathing protection with the appropriate tools.
- Always make sure you stand firmly and balanced when you use a power tool. Don't overreach.
- When you have to walk along a piece, make sure your path is clear of things you can trip over.
- Don't push your power tools and bits too hard. If something breaks while it spins ...... could mean disaster.

I didn't actually mean start it here in "Forum Rules, Guidelines, & Suggestions" this is obviously not the right place, but that is the point, there currently is no "right" place.
The comment was more to Shane as a suggestion for another topic area.
But I must admit frank, I've considered your #3 myself.
