I need to re-do mine on my PM66.  They have seen better days.

Should I use ply with formica, hdme, or just some ply?

How do I use a vertical feather board on one of these?  Screw something to the top so I can attach something?

Those grip tite things?

I don't think I would use ply...maybe as a sub fence that's removable...probably HDME if it were me!

I should have said baltic birch ply.

It has ply with formica facing on it now.

Whatever I use, I am not going to screw it back on.  I think I will use carpet tape.

Darcy, I would not recommend using carpet tape, but that is up to you.  
Below is a picture of how I replaced the faces on a saw I restored a couple of years ago.
On this one. I used 3/4" thick walnut screwed to the fence with the holes left exposed.
The owner of the saw has not had any problems with exposed holes.

What is wrong with carpet seam tape Chris?

Hard to get off?  It sticks like the dickens.

Like I said it is up to you.

I have found it works great for temporary things, but I have found that for long term things, it starts coming loose over time.
When it starts to come loose, sawdust works its way between the loose tape & part it was stuck to & over time causes an uneven surface & more separation.

If its anything like my Biese fence, its gonna need some shimming to get it square to the table (vertically), however you attach it.
