I'd appreciate some advice on connecting a CTL Midi to the split hose on a Festool Router Table.

The Midi has a fixed hose with a (approximately) 35mm outside diameter. But the split hose for the Festool Router Table has a much bigger diameter hose (approximately 57mm).

What do I need to connect the two?

You need this: Adapter hose AD-D36/CS50/CS70/CMS

The Midi's hose is not fixed, you can pull it out of the top of the Midi (this might be a bit tight at first). Insert the adaptor and then connect the Y-split hose to the adapter.
Just pull out straight? I've tried it once but was afraid I'd break it. But I guess I needed just to use a little more force? Or do you also need to twist and turn it?

You need to grab the black angled hose connector, not the green hose. It is a friction fit, but it can be pretty tight when it hasn't been loosened for a while. Twist and turn can help under these circumstances.

Thank you very much Alex, that's the perfect answer!

I wish Festool included more information like this in their product manuals.
