I'm of the opinion that you don't need the large ugly mushroom head transition. From your photo it appears that you have a nice straight edge on your tile and your wood floor. Rip a piece on the table saw to fit into that gap. Counter sink a 2 or 3 holes in it. drop it in and screw it down. Possible caulk the edge against the tile. DONE.

IF you feel the need to have a lip on each edge to cover the flooring material, then make the piece thicker and wider and rabbet the edges to form the over hang.

Cut a dado in a piece of scrap sheet goods to accommodate the center, raised section. 

Use double-sided tape to secure the transition to the dadoed board.  Rip both at one time.  Remove and use.
