Looking for a 4'x4' sheet if anyone knows where I can purchase this in the US.

You might call a local cabinet shop and ask if they've got any Arauco Trupan MR or if they can buy you a sheet and cut it in half. The box stores are usually able to special order these sorts of things too but it'll be a full 49"x97".
 
Just a caveat - there's a big difference between 'water-resistant' and 'waterproof'. I use plenty of green MDF, but only because it machines better, it takes paint better, and it doesn't fur up along the board edges. But it will still swell and fall apart if it gets wet regularly - it just takes longer to fail. The only truly waterproof version of MDF I am aware of is Tricoya;

Anyways to the OP, Sierra FP is in your neck of the woods.  You can search their product listing for water resistant.  Armorite (outdoor), Medex (indoor), and the aforementioned Tricoya (outdoor) are your options there.
 
The quality of MDF has taken a plunge lately in my area.  It used to be that the flat surfaces of MDF would have a glass-smooth super calendared surface. 

It used to be that the only surfaces that needed sealing before painting with water-based paints, were the machined edges.

But that is not the case anymore.  The flat surfaces are not quite smooth, and will “raise the grain” if I paint them with water based paints. 

Additionally, the better lumber dealers are not carrying MDF at all.  They tell me that they cannot make money on them and they are too easily damaged in transit. 

Lowes, Home Depot both carry 1/2” thick and 3/4” thick in 49” x 97” sizes.  The extra inch so that they don’t have to discount for the damaged edges. 

But neither is carrying 1/4” in those larger sizes, just in 24” x 48” sizes.  Those sizes never seem to make economic sense to me.  Too much scrap, too high a price per square foot.

1/4” this MDF bead board is cheaper and if the design allows, I will use that instead.

MDO (Medium Density Overlay) has been the substrate of choice for sign makers for years.  It is good for exterior applications. 
 
MDF hasn't changed per se.  It's just more stratified by grade and purpose now.  Heck, Medite has at least 7 variants now for instance.  You're probably grabbing econo from the box stores as that's the cost point most DIYers will tolerate.  Same with power tools.  Same with everything.

Every manufacturer has a chart these days laying out use cases and more importantly a comparison of the MR, strength/weight, machinability, insect/rot resistance and the NAUF status.

 
You might try Distributor Service Inc.  They have locations throughout the Mid-West.  You can call them at 800.745.1778, be aware that they are a wholesaler so you need to have a business name; they also sell only full sheets.  The Cincinnati location carries Extira, which is an MDF product designed for outdoor use.
