I glued a 2.5" and 1" sheet together for all 3 layers. 10x15 inch. Corners cut off and cutout at back so each layer would fit as the bottom layer based on systainer dimensions.

Looks nice, Can we see some more pictures, please? I might need to steal this idea and modify it...
 
Not sure what else to photograph but here is one with everything stacked up:

I make it a habit to have the manuals and other relevant information (like great articles from this board I printed out) in a zip-loc bag in the systainer for easy reference.

So you cut the thicker piece before gluing the bottom on, correct? How did you cut the top piece? It looks really clean and 'factory'.
 
Nope I glued everything together first. Used a foam marker to do the outline and a narrow utility knife where I extended the blade to the proper depth of cut. It is certainly easier to do a shallow cut first and than work your way down to keep cutting down straight.

After thats its digging in with your fingers at an edge and ripping the foam out carefully. Larger pieces I cut up in 2 or three sections to make removal easier especially if you have a deep section.
