I just received the interface pad for my ets150 (it's a foam pad that attaches to the sander with hook and loop) to use on a shield that my son and I are making for him.  The shield is curved so I figured this would be good to use to sand the inside contour since the pad on my sander lacks the flexibility.  I used it for about 5 minutes before it started to come off.  I took it off, vacuumed the sander pad and the interface pad and then reattached it.  Now it won't stick on at all when I start the sander, it just gets thrown off.  The interface pad has the new hole configuration and I'm not sure if that has anything to do with it but I bought one of the hard pads with the new hole configuration awhile back and had the same issue.  It worked for a few minutes and now it won't hold the sandpaper that I have that has the old hole configuration.  Any ideas?
 
Just a suggestion, but are you using Festool disks?  if not, I know that Abranet disks destroy the hook and loop, so you need a thin interface pad to protect the sander pad.  Look at the hook and loop with a magnifying glass to see if it's still intact.

I'm using Festool paper.  I don't think the pad is shot.  That certainly wouldn't explain why I had the same problem with the hard pad that I bought a few months ago.
